By completing and submitting this form, you confirm that you have reviewed and understand the LCS Simplified Franchising Agreement.  You also verify that your information is accurate and true.  Upon receipt, LCS will review your application to consider your qualifications.  If you appear to be qualified for a local franchise in your immediate location, you will be contacted for a personal telephone interview.  Not all applicants will qualify and may not be contacted.  Please complete the entire application form below.
