By completing and submitting this form, you confirm that you have reviewed, understand, and accept the LCS Simplified Franchising/Affiliate Agreement.  You also verify that the information you submit is accurate and true.  You are not an employee of LCS (Local Computerized Services, LLC).  Upon receipt, LCS will review your application to consider your qualifications.  If you appear to be qualified for a local outlet in your immediate location, you will be contacted for a personal telephone interview.  Not all applicants will qualify and may not be contacted.  Please complete the entire application form below.  Thank you.
